Constipation & Incontience

Since there is no single definition of constipation, its definition has been tried to be made on the basis of the findings. Criteria such as the number of poops per week, pain during defecation, pain, bleeding, delaying the poop patch, whether the stool diameter is large or small to force defecation, poop incontinence, bleeding with defecation, and pooping that will cause toilet blockage are used in the definition of constipation. If at least 2 of the listed complaints persist for more than 1 month, it can be considered as constipation. Therefore, another important point is whether the patient's complaint is constipation.

There are difficulties in the treatment of constipation, as in its definition. It should be kept in mind that the age of the patient presenting with the complaint of constipation is in the first place. The duration of the complaint and accompanying findings should be evaluated in detail, starting from the prenatal period. Causes that may cause constipation can be endocrine causes such as hypothyroidism, electrolyte imbalances, intestinal diseases, side effects of drugs, surgical diseases (cases born with an anus in place or closed, Hirschsprung's disease).

Stool or poop incontinence is possible with the organized work of the anal region muscle complex with bowel movements. Incompatible work can result in constipation, incontinence. Even urination problems can be seen together. In cases where no underlying cause could be found despite all investigations, functional constipation can be concluded, which is the most challenging group for us in treatment.

Treatment:

  • Difficulties in diagnosis are also experienced in treatment. If an underlying cause can be identified, treatment for the cause is applied. The main problem is the constipation group, whose underlying cause cannot be determined and who have constipation complaints. For this group of patients, it is possible to be successful with drug treatments according to age, complete evacuation of the intestine (sometimes mechanically or with drugs under anesthesia), behavior and nutrition arrangements, and close follow-up.
